3.5 Fundamental Management of Congestion (Fig. 3.7)

In most patients with HHF, congestion is a major pathophysiological condition. If congestion is sustained or worsens, it will be deleterious for organ function, including cardiac function. Several studies have demonstrated that the time from symptom onset to hospital arrival or treatment is closely related to the patient’s outcome. Analyses from the Tokyo CCU network revealed that time interval from onset to hospital care [47] or the transportation time [48] was associated with the severity of HF and in-hospital mortality, suggesting that a shorter time to treatment of HF is associated with better in-hospital outcomes. Furthermore, several studies have demonstrated that the time to treatment is closely related to in-hospital mortality in patients with HHF [49–51]. Based on these investigations, recently the practical guidance has been published to improve the outcomes of patients with HHF [52]. Figure 3.7 shows that modification of treatment according to the patient’s continually changing pathophysiological condition is important.
